How do u know what is wrong when your power steering fluid is leaking?

I have a 1996 town and country minivan and it leaks out power steering fluid. I have to put it in there probably once every 2 weeks maybe more. Can anyone please tell me what I need to do to fix this or if I can do it myself. Thanks Tracy
